Please submit your sealed Quotation, together with the Request for Quotation (RFQ) and Quotation Submission Sheet (QSS), to the Procurement Office, Old SDO Building, Dapitan City Central School, Sta. Cruz, Dapitan City, on or before the prescribed date and time indicated in the RFQ/QSS.

Please be advised that quotations received after the specified deadline shall no longer be accepted.
